App-V 5.0 SP3 Sequencing ConfigMgr 2012 R2 Admin Console

Most of sequencing this app is straightforward. However, you need to specify the primary virtual application directory (PVAD) when running the sequencer. In order to do this, launch the sequencer from the command line with -EnablePVADControl (I’m running this on Windows 10 1607 where App-V is now part of the Windows 10 ADK).

C:\Program Files (x86)\Windows Kits\10\Microsoft Application Virtualization\Sequencer\Sequencer.exe - EnablePVADControl

Or you can change this registry key to 1 always launch the PVAD UI.

HKLM\Software\Microsoft\AppV\Sequencer\Compatibility\EnablePVADControl

Also, select to allow the app to write to the virtual file system.

That should be it.
