Breast augmentation or breast enhancement is a procedure done to increase the size of the breast. It is performed for women who have lost fullness after pregnancy or breastfeeding, and also for women who desire more volume. It is a popular method used to increase the size, augment the shape of the breast and improve the symmetry of a women’s breast. This is most often done using silicone cohesive-gel implants, saline implants and sometimes fat-grafting.

Breast augmentation is a signature procedure at Allure Plastic Surgery. It is done safely under sedation where the patient is asleep throughout the procedure. The patient is able to go home the same day with minimal downtime.

Breast Augmentation -Tear Drop Implant

Choice of implant:

Breast implants serve as a breast filler that's either filled with silicone implant (cohesive-gel) or saline. The shell of the implant may be smooth or textured. Silicone breast implants also come in either round or tear-drop shapes and patients are able to opt for breast sizes down to the cup size. Implant size and type choices have their pros and cons, hence a detailed discussion with the surgeon is conducted before a final decision can be made.

Choice of incision:

The implant may be inserted through an inframammary fold incision (in the fold under the breast), a peri-areolar incision (around the nipple) or a trans-axillary incision (underarm). The final choice on the type of incision used is based on patient wishes, whether the surgery is first-time or a revision, and the size and shape of the implant.

Choice of pocket:

The implant can either be placed:

  • In a pocket under the breast (Subglandular)
  • In a pocket under the pectoralis major muscle (Subpectoral)
  • In a pocket where the upper portion of the implant is placed under the pectoral muscle, while the lower half of the implant sits directly underneath the breast tissue (Dual plane)

A subglandular placement technique can provide a small lift and create a pronounced cleavage. There is also no risk of distortion when flexing. On the other hand, a subpectoral implant technique allows for a less defined and more tear-drop shape with more tissue coverage.

The dual-plane implant placement is most preferred as it offers the benefits of both the aforementioned techniques. Additionally, it also gives the most natural-looking appearance and offers a good breastfeeding success rate.

Ultimately, the type of breast implant placement should depend on a variety of factors, such as your desired look, your body type and existing breast tissue and the size and shape of the implants you have chosen.

Breast reconstruction

Reconstruction of the breast may be required after removal of the breast (mastectomy), most commonly due to cancer and less so due to trauma or severe infection. Congenital conditions may also necessitate breast reconstruction. Mastectomy may be partial or total.

Reconstruction may be carried out at the same time as the mastectomy, termed primary reconstruction, or at a later stage, termed secondary reconstruction. This can be achieved through an autologous technique using tissue from either the patient’s tummy (preferred) or the patient’s back, or by using an implant prosthesis.

Breast reconstruction is a medical procedure, hence Medisave and insurance claims are possible.

Breast Fat Grafting

Breast Enhancement using Fat-grafting or Lipo-filling

Fat grafting to the breast can be performed in order to augment the size and symmetry of the breast. The fat is harvested from the patient’s tummy, thighs or buttocks, and may be done as part of a liposuction procedure or alone. The fat is then carefully processed using a well-established protocol and injected into the breasts. Staged procedures may be required to achieve the desired outcome. This is usually performed 3 months apart.

Fat grafting to the breast may also be performed for medical reasons, such as to correct deformities in the breast due to removal of large breast lumps or previous breast infections. Fat transfers involve delicate work with the breast tissue or even the chest muscle and must only be performed by a certified plastic surgeon. What is the recovery time to expect?

Breast augmentation at Allure Plastic Surgery Singapore is carried out on an outpatient basis, meaning that there is no need for an overnight stay. Patients are given general anaesthesia and will be asleep through the procedure.

Patients experience some swelling and bruising but you will be able to return to work on day 2 or day 3. There are usually no visible scars.

What is the difference between breast augmentation and breast implants?

Breast augmentation is the term of the procedure that can be done with implants (silicone or saline) or fat graft. In short, breast implants is just one of the various methods by which breasts can be augmented, or made bigger.

How much does breast augmentation cost in Singapore?

At Allure Plastic Surgery, the estimated cost for breast augmentation is around $15 000 to $18 000 all-inclusive. The cost in Singapore varies based on a few different factors. These can include the method of choice (fat transfer vs breast implants), the types and brands of implants used and even down to the experience and reputation of the plastic surgeon itself.

Do breast implants feel different to touch?

Implants generally feel very firm during the first 6 months post-op. While this will not feel the same as natural breasts, implants gradually soften as time goes by. After about a year, it will be difficult to feel the difference between real breasts and implants, especially if the native breast tissue isn’t thin to start with.

How long does a breast augmentation last?

Breast augmentations are permanent and last a lifetime. With the newer implants we use at Allure Plastic Surgery, there is no longer a recommendation to change your implants every 7-10 years. Newer implants are very medically advanced and have a lifetime warranty against cracking or leaking.

What's the safest breast implant?

While the biggest contributing factor to the safety of breast implants is an experienced surgeon, Allure Plastic Surgery generally recommends smooth or nanotextured cohesive gel implants for the highest safety.

Which type of breast implant feels the most natural?

As a plastic surgeon, my view is that soft, smooth surface, cohesive gel implants feel the most natural and are almost indistinguishable from real breasts if the procedure is done well.

Do breast lifts require implants?

Breast lifts don’t always require implants. It differs based on what the patient wants to achieve and her current situation. As an alternative to implants, I also often use fat grafting in breast lift procedures. I’ve also worked on multiple cases where the native breast volume is enough such that neither implants nor fat grafts are required. Having said that, breast implants do give a good balance between the length of scars on the breast and the amount of lift and shaping achieved.

Can I get breast augmentation without implants?

Yes, you can. Breast augmentation doesn’t necessarily need to involve implants. We can also use the natural fat graft method to increase the volume of the breasts and enhance the appearance.

Does a breast lift reduce your cup size?

Breast lifts definitely result in reduced cup size. My patients generally experience a minimum of half a cup size reduction. However, don’t be alarmed as the resultant lift compensates for this reduced cup size in appearance. Droopy breasts look smaller than similar volume, perky breasts. There are also methods to maintain the cup size after a breast lift, and I advise these on a case-by-case basis during consultation.

Is 50 too old to get breast implants?

Is it better to get breast implants under the muscle or over?

My opinion as a plastic surgeon is that neither under nor over muscle implants are optimal. Given the choice, I usually recommend my patients the dual plane placement as it provides a better lift, is less likely to sag and is less likely to be felt through the skin. What happens if you don't replace breast implants?

The need to replace implants every 5-7 years is old information based on outdated medical science. Newer implants are permanent, lifetime warrantied against cracks and leaks and no longer need routine replacement.If you’re using old implants right now, it might be recommended to have them removed and replaced with fat grafts. What happens to breast implants when you're old?

Many of my patients ask this question. Newer implants last a lifetime and will maintain their shape even when you’re old.

How many cc is a size C implant?

There is no fixed cc for a specific cup size. A lot depends on the original breast size and the lady’s rib cage circumference. If you’re specifically looking to get cup C size, it’d be better for me to advise on the cc and implant size after analysing your body shape and structure during a consultation.

When should I start to massage my breast implants?

It’s important to massage your breasts after doing implants. I recommend my patients begin this 1 week after the surgery. I will provide more specific directions during post-op consultation.
