How to find the Phantom Helmet
Video Guide
Misko leaves the following note about the helmet:
"Find these pieces at a battleground where brave souls tested their skill."This clue leads Link to the Coliseum Ruins, north of the Great Plateau and south of the Mount Daphnes. You'll need to enter and defeat (or avoid) the Lynel inside, then look along the northwest walls of the ground floor and use Magnesis on the buried chest to find the Phantom Helmet.

Misko leaves the following note about the armor:
"at ceremonial grounds where royal guards are honored."Remember the ceremony back in one of the memories? You'll find this item in the Sacred Ground Ruins, south of Hyrule Castle Town Ruins and northwest of Mabe Village Ruins. Once at the ruins, check the middle ring of water along the southeast side to use Magnesis to pull the chest out of the rubble. Inside you'll find the Phantom Armor.

Misko leaves the following note about the greaves:
"at the ruins where the soldiers of Hyrule gathered."Find this location by going to the Hyrule Garrison Ruins, east of Mount Daphnes and north of Windvane Meadow. Look for a rusted Guardian on the eastern edge of the ruins and use Magnesis on the chest below it to find the Phantom Greaves.
Phantom GreavesAnd there you have it! The very cool Phantom Set gives you a nice attack bonus and is inspired by Phantom Hourglass and Spirit Tracks.
