This page details how to access the Bloodborne: The Old Hunters DLC.

Once you have the DLC, the first thing you'll need to pick up is the "Eye of a Blood-drunk Hunter" that can be found near the Doll in Hunter's Dream. However, the item will not spawn unless you have defeated Vicar Amelia and interacted with the skull on the altar.

Once you have gotten the "Eye of a Blood-drunk Hunter", the player must head to the Cathedral Ward, head out the left door, and go outside to where the Lesser Amygdala is located, by the corpse near a dead tree. Walk over to the corpse, and let yourself be grabbed by the Lesser Amygdala. You won't die, I promise. Once you've done that, you'll be transported to the Hunter's Nightmare, where you can finally start playing the DLC
