What is an Intra-Uterine Device (IUD)?

The intra-uterine device (IUD) is long-term, reversible mode of contraception. It is one of the most effective birth control methods available, with an over 98% success rate at preventing pregnancy.

The IUD is a tiny T-shaped device attached to a string, and it is inserted into the uterus, where it sits and prevents pregnancy. This insertion process can be carried out relatively quickly and easily in the clinic.

Copper IUD and Hormonal IUD

There are two types of IUDs

There are two types of IUDs available- the Copper IUD and the Hormone Eluting IUD.

Depending on the type of IUD, it can last anything from between 2.5 to 10 years.

The IUD is suitable for both women who have given birth before and women who have never conceived.

It is a safe and convenient option for someone who is not looking to conceive in the immediate future.

It lasts for years and is easily removed in the event that you wish to have a baby.

Frequently Asked Questions (FAQs) on IUD

How does the hormonal IUD work?

The hormonal IUD is a soft, flexible plastic device which contains the progestin hormone and releases small amounts of it gradually and regularly into the walls of the uterus.

It prevents pregnancy through several effects:

• by thickening of the cervical mucus at the entrance of your womb to form a barrier that prevents sperm from passing through
• by inhibiting sperm from reaching the egg
• by thinning the lining of the womb which makes implantation of a fertilised egg unfeasible.

How does the copper IUD work?

Nova-T prevents pregnancy by inhibiting the movement of sperm cells so they cannot meet and fertilise the egg. The copper content has a toxic effect on sperm cells.

How long does the IUD last?

Hormonal IUD lasts 5 years and Nova-T lasts 2.5 years, after which removal or replacement is necessary.

How efficient is the IUD at preventing pregnancy?

Both the hormonal and copper IUDs are >98-99% successful at preventing pregnancy. In comparison, condoms are about 85% effective.

How do I know if the IUD is suitable for me?

The IUD may be a good option for you if:

• you are looking for a birth control method that is convenient and which you do not have to think about daily
• you do not intend to conceive in the next few years but may eventually want children at a later date

The IUD may not be a suitable option for you if:

• you have or previously had a serious pelvic infection
• you have cancer of the womb or cervix
• you have a condition that changes the shape of the inside of your womb e.g. a large fibroid

What tests do I need to do before I get the IUD?

It is best if you have done your regular PAP smear prior to getting an IUD.

If there is any reason for you to worry about a vaginal or sexually transmitted infection, it is best that you test for and treat this before getting an IUD.

What is the IUD insertion process like?

Insertion of the IUD is best done during your menses as this is when your cervix, the entrance to your womb, is most relaxed.

During the insertion of the IUD, a speculum is inserted to open up the vaginal canal. The cervix is then cleaned, and a metal sounder is gently inserted through the cervix opening and into the womb to determine the depth of your womb and whether the IUD can be inserted.

Your doctor may have to use a small pair of forceps to hold your cervix in place.

The IUD is then inserted into your womb and the strings will be cut. You may experience a little cramping and discomfort, as well as some minor bleeding.

About 2-4 weeks after insertion of the IUD, you will have to do an ultrasound scan to confirm the placement of the IUD.

What are any side effects or procedural risks during insertion of an IUD?

Some degree of bleeding/spotting, cramping and discomfort are expected side effects during insertion and may persist for several days after.

There is also a possibility of failure of insertion, which could be due to various factors.

The IUD passing through the wall of the womb is an extremely rare but serious risk.

What are the common side effects of the hormonal IUD?

During the initial few months after insertion of Hormone-releasing IUD, you may experience spotting in between your menses and slightly irregular menstrual periods.

Over time, your period will get lighter and up to 20% of women with hormone-releasing IUD may have no menses altogether.

What are the common side effects of the copper IUD?

Heavier menstrual periods and potentially more cramping can be expected with the copper IUD, hence if your period is already significantly heavy, the copper IUD may not be ideal for you.

What are some of the risks involved with an IUD?

The IUD may infrequently be expelled from the womb/may fall out by itself, in which case, alternative backup birth control methods or emergency contraception may be required. Infection may occur in rare cases as well.

There is a small risk that the IUD may get embedded in the wall of the womb or, in extremely rare cases, puncture the wall of the womb and pass through completely.

Will my partner be able to feel my IUD during sexual intercourse?

Your partner will not be able to feel the IUD during intercourse. If your partner can feel the IUD strings during intercourse, you can speak to your doctor who will check if they need to be trimmed.

How do I remove my IUD when it is due?

Removal of the IUD is simple and can be done quickly in clinic. Your doctor will insert a vaginal speculum, locate and grasp the strings of the IUD and gently pull it out.

Can I get pregnant immediately after removing my IUD?

Yes, you may be able to conceive soon after removing your IUD. Return of fertility is fairly rapid.

Up to 80% of women manage to conceive within a year after removal of their Hormonal IUD.

For the copper IUD, return of fertility is immediate from the point of removal.
