As you approach him and speak with him he'll ask for a "pure white flower." This refers to a white Pinwheel found within the Senpou Temple area.
Where to Find the PinwheelsHe's referring to pinwheels and you'll find two of them along the paths and ledges of Senpou Temple. You'll make your way through the area and after finding and resting at the Shugendo Sculptor's Idol, you'll grapple ahead where you'll find 4 Senpou assassin enemies to dispose of.
You'll reach a point where you can head up to find a bridge with the Mini-boss the Armored Warrior, or you can grapple forward and grab a ledge mid jump. This path will lead you to the White Pinwheel, and a shortcut to the Demon bell Sculptor's Idol you may have found earlier in the game.
You can head to the bridge where you'll find Armored Warrior, but head the opposite direction to find tons of tiny statues and a Red and White Pinwheel.
Spirited Away (Happy Ending)When you return to Kotaro and give him the White Pinwheel, he'll ask to "spirit" him away.
To do so you'll need to use the Divine Abduction prosthetic tool. This can be found in the Sunken Valley. After defeating the Long-Arm Centipede Giraffe mini-boss, you'll find this prosthetic in the same room.If you have the Divine Abduction prosthetic tool equipped, use it twice on Kotaru to spirit him away. Then, once you've unlocked the Hall of Illusions, you'll meet him again to finish his questline.
After defeating the Folding Screen Monkeys, you'll unlock the Inner Sanctum. If you completed the previous steps, Kotaro should show up in the Hall of Illusion, and will reward you with two Taro Persimmon items.
Anayama and Abandoned Dungeon Endings (Sad Endings)There are two sad options for the end of Kotaro's story. Let's start with the saddest one.
Abandoned Dungeon Ending
Upon first approaching Kotaro, it's possible that players may think he just wants to die. Unfortunately, there is a way to have the poor guy suffor. If you met the NPC Doujun in the Abandoned Dungeon, you'll know he's looking for a "tough man." Several NPC's in the game fit this bill and can be sent to him, including Kotaro.
Give him the Red and White Pinwheel, and if you've visited Doujun already, you'll have a chance to send Kotaro to the Abandoned Dungeon. Once you've done so, Doujun will give you 3 Lumps of Fat Wax. Once getting the dive technique, you'll be able to face the poor man you imprisoned.
Kotaro and Anayama
Early in the game, you probably met Anayama the merchant near the Outskirts Wall - Staircase Idol in the Ashina Outskirts. If you've completed his questline up to a certain point, he will ask for a helper that fits Kotaro's description.
Give him the Red and White Pinwheel, and you can send him to Anayama if you've gotten far enough in his questline. Visiting Anayama, you will see that sending Kotaro to help him has made the merchant quite lucrative, and he has added some new upgrade materials to his wares including lumps of fat wax, yellow gunpowder and scrap magnetite.
