• Lucky for Life costs $2 per wager.
• Fill in a Lucky for Life playslip by choosing 5 different "white" ball numbers from 1 to 48, and one "yellow" Lucky Ball number from 1 to 18. Or, ask for a Lucky for Life “Quick Pick” and let the Lottery terminal randomly pick your numbers.
• Lucky for Life tickets cannot be voided; once printed there is no way to cancel them. Please be sure to check your ticket carefully after purchasing to make sure it is what you requested.
• CLICK HERE to check if your Lucky for Life numbers have ever been drawn.
• ADVANCE ACTION: Buy tickets for future Lucky for Life drawings with the ADVANCE ACTION option. Play up to 28 consecutive drawings in advance. When playing ADVANCE ACTION, fill out only one board (set of numbers) per playslip. (i.e. To play four sets of numbers in advance, fill out four separate playslips.)
• REPLAY: To play your numbers again, hand the Lottery Retailer your existing Lucky for Life ticket and ask for a REPLAY. You will get a new Lucky for Life ticket with the same numbers and wager amount. Your new ticket will have the REPLAY “R” printed on it.

• 1 top prize winning ticket: $365,000 a YEAR FOR LIFE (Annuity) or $5,750,000 (Cash Option)• 2-14 top prize winning tickets: The prize value is split equally among the winning tickets; the minimum prize value for this category is $500 a week for life.• 15 or more top prize winning tickets: The top prize liability of $7,125,000 is split equally among all top prize winning tickets and paid in one lump sum payment. The minimum prize value for this category shall not be less than any lower tier prize paid in that respective drawing. Minimum top prize period is 20 years. Top prize winners may choose the Cash Option as an alternative to the Annuitized Payment Option. Top prize amount shown is based on one winning ticket. The "For Life" prizes are guaranteed for a minimum of 20 years. The lump sum option for the Top Prize is $5,750,000 (before taxes), and for the Second Prize, it is $390,000 (before taxes). What happens if an annuity winner dies before receiving the minimum prize payout? In the event of an annuity prize winner's death, the Lottery will make any remaining guaranteed payments to the winner's estate or beneficiary as directed by court order or other governing document. No. Although it is a multi-state game, Lucky for Life tickets must be claimed in the same state or jurisdiction in which they were purchased.
