Jack is a squadmate in Shephard’s party in Mass Effect. In the Suicide Misson, there is a possibility that many of your squadmates could die. Jack is no exception as she can die in the mission as well. After preventing her death in the Suicide Mission, she will live to come back in Mass Effect 3. How to Save Jack in Mass Effect 3

You will get a mission at Grissom Academy after completing the quest Priority: Palaven. Accept it as this will allow you to meet Jack. You can meet her at the Purgatory bar on the Citadel. The mission can go away if you do not go to Grissom Academy soon.

After this meetup, the next time you see her, she will be your enemy. When you raid Cerberus Headquarters, she does not seem to be herself and will appear in the form of a Phantom. You will have to take her down and her misery. You also find an audio log that clarifies that Jack resisted Cerberus up until the very end. Shephard takes upon him to avenge her as well as the other crimes done by Cerberus.
