Spider-Man: Every Time Aunt May Figured Out Peter Parker's Secret ...

One of the longest-running aspects of the Spider-Man franchise has been the young hero trying to keep his identity a secret from his beloved Aunt May. Over the years, her discovering his identity has been played for drama, not just in the core-Marvel Universe but in other universes and mediums as well.
Here are all the ways Aunt May has found out that Peter Parker is Spider-Man.

AMAZING SPIDER-MAN #400
After suffering a serious stroke, May is brought to the Forest Hills hospital -- with Peter arriving shortly after in Amazing Spider-Man #400 by J.M. DeMatteis, Mark Bagley, Larry Mahlstedt and Randy Emberlin. Although she recovers quickly from the sickness, Peter continues to worry about her health. Eventually, the pair go to the Empire State Building and overlook the city -- which is where May reveals that she's known Peter's secret identity for years. Before Peter can learn more, May collapses into his arms. As her heart gave out, Peter gets her home in time so she can seemingly pass away peacefully in her bed, taking the secret to the grave.
However, there were far more mysteries at the heart of May's "death." It's eventually revealed that this woman wasn't in fact the real May Parker. In reality, the real May -- who'd never known Peter's secret identity -- was captured by Norman Osborn and replaced by an actress who died in her place. Peter would later discover May being held prisoner and would free her. As the actress had only learned Peter's identity from Norman, May remained unaware of her nephew's alternate identity.
AMAZING SPIDER-MAN #37-38
In Amazing Spider-Man Vol. 2#37 by J. Michael Straczynski, John Romita Jr and Scott Hanna Aunt May visits Peter and finds him passed out in his bed after a recent battle with Morlun -- still in costume, accidentally exposing his identity to her. May took a day to process this information, eventually confronting her nephew in the following issue. After the two open up about the reasons why Peter became Spider-Man, May learns to accept her nephew's position as a hero -- even helping him when he moved into the Avengers Tower.
May eventually lost her knowledge of Peter's identity the same way the rest of the world lost that information: the events of One More Day. Spider-Man exposed his identity to the world, including May, during Civil War. After being shot during an assassination attempt on Peter, May was left on death's door. Rather than let her die because someone was gunning for him, Peter and Mary-Jane make a deal with Mephisto. They trade their marriage, creating a shift in reality that eliminated their full relationship. In exchange, May's life is restored and the entire world -- including her -- forget that Peter is Spider-Man.

ULTIMATE SPIDER-MAN #99-100
One hundred issues into Ultimate Spider-Man by Brian Michael Bendis and Mark Bagley, Peter Parker found his identity exposed to his Aunt May during the bedlam of his version of the Clone Saga storyline. May is furious with this revelation, even briefly kicking Peter out of the house for hiding these secrets from her. But when her nephew and Sue Storm are able to save May's life after she has a heart attack, she comes to terms with Peter's alter ego. She even opens her home to other young heroes, including Human Torch and the Carnage-version of Gwen Stacy.
Unfortunately, this version of Spider-Man would later have his identity revealed in the most brutal way possible. After taking a bullet meant for the Ultimate Captain America, Spider-Man was chased down and confronted by his greatest enemies. Eventually worn down, Peter Parker was finally killed, and his identity revealed to the world at large -- meaning that May wasn't a secret-keeper for very long. However, his memory did inspire Miles Morales to take on the mantle of Spider-Man, eventually becoming a multiversal hero in his own right.
THE MARVEL CINEMATIC UNIVERSE
May Parker is still the chief caregiver for Peter Parker in the Marvel Cinematic Universe. She first appeared in Captain America: Civil War, played by Marisa Tomei. She's since appeared in the two following Spider-Man films, Spider-Man: Homecoming and Spider-Man: Far From Home. At the conclusion of Homecoming, she discovers Peter in his costume inside his bedroom, similar to how she discovered him on Earth-616.

Both her and her nephew were among the people who were snapped out of existence as a result of Thanos utilizing the Infinity Gauntlet to wipe out half of all life in the universe. Both were then restored when Hulk used the time-displaced Infinity Stones to bring everyone back to life in Avengers: Endgame, May's knowledge is still in-tact. However, at the conclusion of Spider-Man: Far From Home, the last actions of Mysterio revealed Spider-Man's identity to the public.
