I hear a lot of puerto ricans say it. Is it "you already know"? Or "now you know"? Or both ?

What this means is whoever is saying it does not have a very good command of the language.

Yes, it can be "now you know," but the personal pronoun is not necessary. It can be used for emphasis though to identify "you" from other people present, but like this: "Ya sabes tú." There are many times the personal pronoun can be in different places of a sentence, but in this case, it sounds really strange before the verb.

It can also mean "you already know," but again with a different placement of "tú." In this situation, it´s most common to say "Ya lo sabes (tú)" and the pronoun "lo" is many times used, depending on the context.
