We are led to believe she died, but the only time Opie and Andy actually discuss her is during an episode where Andy is trying to explain "love" to Opie. Saying that when you marry you have that deep down special feeling. Opie asks, "Did you have that feeling for Mom?" and Andy says, "Yes son, I did." That is it.
