What we do
The Community Therapy Service (CTS) provides short term rehabilitation at home for people who are having difficulty in managing their normal daily activities following a recent illness or injury.
CTS aims to increase/maintain a person’s independence and quality of life following the changes in their health.
Who is the service for?
CTS is available for adults with a new health condition affecting their quality of life.
You must be over 18 years old and registered with an Oxfordshire GP.
Those needing the support of a CTS Mental Health practitioner should be over 65 years or have a diagnosis of early onset dementia.
What should you expect?
CTS will complete an initial assessment of your needs either via telephone or video consultation. If neither of these options are suitable to you then a face-to-face visit can be arranged.
They will assess your needs and create with you, a personal care plan and treatment goals. These will follow a SMART principle:
- Specific
- Measurable
- Achievable
- Realistic
- Time-bound
Once your goals are established, they will work with you for a time limited course of intervention to help you achieve your goals. Regular progress review will be offered by various members of the CTS team.
Interventions
Interventions can include:
- Provision of basic equipment
- Home-based exercise programs
- Collaborative problem solving
- Practicing activities to build confidence with new strategies
All interventions will be personalised to your needs.
Our team
Each CTS team is made up of the following health care professionals:
- Occupational Therapists
- Physiotherapists
- Mental Health Practitioners
- Assistant Practitioners
- Rehabilitation Assistants
- Administrators
- Students
Each member you need will work with you to help you achieve your goals.
CTS also works closely with other health, social and voluntary services, offering appropriate onward referrals and signposting to relevant services as required.
Non-urgent advice: How do I refer?
Call the Single Point of Access for Community Services on 01865 903 750 and they will take your details and pass these on to your local CTS team who aim to get in touch with you as soon as possible to make the initial assessment appointment.
Referrals can be made by anyone who has gained consent from the individual being referred.
