The forms are ONLY available and accessible through the Department of National Defence and the Canadian Armed Forces intranet (DWAN) at the homepage link provided below on this page. It has come to our attention that clients visiting the Defence Forms Catalogue(DFC) are getting errors and cannot access the DFC. The error is intermittent and SSC has been made aware of the issue. We ask everyone with difficulties accessing the Defense Forms Catalogue to submit an assyst ticket on the DWAN. Below are the instructions: 1 – Go in assystNET - Services (mil.ca) and type in search bar ‘ report an enterprise application incident 2 – click on ‘Report an Enterprise Application Incident Report’ fill in the Following information: a) Type in Defense Forms Catalogue (DFC) and click on magnifying glass; then select Defense Forms Catalogue and click ok b) In description please explain error you are getting and the solution to remove cache through browsing history not working. c) Asset Tag or Application name leave blank d) Please save print screen of error message from DFC on to your desktop and attach to assyst ticket.
