Forms for Voter Registration

Procedure for obtaining certified entry of Electoral Roll

You can obtain a copy of the certified entry by approaching the Electoral Registration Officer (ERO) concerned. You will be required to deposit the requisite fee and thereafter the ERO will issue the required certified copy.

Form Name	Form Description	Download
Form 6	For inclusion of name for the first time and also for inclusion of name of a person already enrolled in a constituency for inclusion of name in another constituency. This form should be submitted to ERO concerned.
Form 6A	Registration of overseas electors.
Form 6B	Letter of Information of Aadhaar number for the purpose of electoral roll authentication
Form 7	Voter Application Form for Objection for Proposed Inclusion/ Deletion of Name in Existing Electoral Roll
Form 8	Voter Application Form for shifting of Residence/Correction of Entries in Existing Electoral Roll/Replacement of EPIC/Marking of PwD
Form 001A	For submission of photograph(0ptional) along with form 6/8.
Form 001B	For optional submission of photograph for photo electroral roll & elector photo identity card.
