Breast Reconstruction

Breast reconstruction is surgery to restore the appearance of the breast, usually after a mastectomy. The choice of how to proceed after the removal of a breast is a deeply personal one, with some women opting to wear a prosthetic form in a bra, and others choosing to embrace their new breastless form as a reminder of their strength and survival. Those options are perfectly acceptable, but many women feel most comfortable with their missing breast tissue surgically replaced. Breast reconstruction can be part of a patient's return to normal after experiencing breast cancer.

There are a few different methods used to create the round shape of the breast on the chest. A flap of fat, skin, and muscle can be taken from the side, abdomen or buttock and implanted on the chest to create a breast from the patient's own tissue. Alternatively, the skin over the chest may be gradually stretched to allow for the placement of an implant. Stretching of the skin is done using expanders, which are similar to breast implants, except they increase in size when saltwater solution is injected into them. Many women also choose to combine the flap procedure with implant placement to achieve their desired result. Once the new breast shape is formed, a nipple can be created and even tattooed to match the other side.

Reconstruction may be done in a single surgery or broken up into multiple procedures:

  • Immediate reconstruction rebuilds the breast right after mastectomy. This method is not recommended if additional chemotherapy or radiation is needed after surgery.
  • Delayed reconstruction occurs after chemotherapy or radiation has been given. These treatments may decrease the volume or alter the color of the reconstructed breast, so they should be completed before the breast is rebuilt. This type of reconstruction may happen weeks, months, or even years after mastectomy.
  • Staged reconstruction splits the reconstructive process into two parts. The first part inserts temporary expanders to stretch the skin and is done immediately after breast removal surgery. The second part replaces the expanders with implants after chemotherapy or radiation treatments have been given.

Breast reconstruction is not perfect. The two breasts may not always look exactly identical, although they will be close. There may be small scars, and a loss of sensation in the reconstructed breast. A reconstructed breast will not produce milk, so patients may have trouble breastfeeding. Still, reconstruction is a great option for women who want that part of their appearance back.

Pressure Ulcers (Bedsores)

Commonly referred to as bedsores, pressure ulcers or pressure sores are areas of injured skin that result from staying in one position for a long period of time. Being bedridden or confined to a wheelchair puts a person at a high risk of developing such sores. The most commonly affected sites are the buttocks, hips, lower back, shoulder blade and spine areas, and backs of arms and legs. A pressure ulcer typically starts out as tender, reddish skin and may turn into a shallow wound that appears like a blister. At the next stage, it becomes a deep wound with yellowish dead tissue and some exposed fat. The most severe pressure ulcers are large wounds with dark dead tissue and exposed muscle or bone. Early detection and treatment are crucial as ulcers tend to develop rapidly and cause infections that could eventually lead to organ failure.

Mild sores may heal on their own when pressure on the affected area is reduced. This can be accomplished by simply repositioning the body every hour. If the sores are painful or infected, medications such as pain relievers and antibiotics may be prescribed. Cleaning the wound and changing dressings regularly also help the healing process. Severe pressure ulcers, on the other hand, may require surgery, which involves removal of any damaged, infected, or dead tissue and closing the wound. Wound closure can be achieved either by stitching the skin edges together or by using a flap of the patient's own tissue.

If a patient needs to have pressure ulcer surgery, they will likely be on bedrest for a few weeks to allow the wound to heal completely. The patient and their family members will be given instructions on positioning and transfer techniques to help keep pressure off the wound. When the doctor thinks the patient is ready, he or she will give the patient special exercises to help them regain their range of motion. To prevent the recurrence of ulcers, patients or their caregivers should be diligent about changing positions regularly, observing for early signs of new pressure sores, and replacing any worn-out mattresses or cushions.

Skin Grafting

When a large area of skin is too damaged to heal, a skin graft may be used to protect the tissue underneath and reduce scarring. A layer of healthy skin is taken and moved to the injured area, where it is fixed in place. Skin grafts are used to treat burns, infections, large wounds, ulcers, and other significant problems that affect the skin.

There are several different types of skin graft, depending on where the donor skin is taken from:

  • Autografts, where healthy skin is taken from another place on the patient's body
  • Allografts, where the donor skin is taken from another person or cadaver
  • Xenografts, which are harvested from animals and are used temporarily like a dressing to cover a wound
  • Synthetic grafts, which are made from non-biological materials, like silicone

In all cases, the skin graft may either be full-thickness or partial-thickness. While thicker grafts make recovery more difficult for the donor site, they heal more easily than thinner grafts.

Before skin graft surgery, patients are given anesthesia so they cannot feel any pain. If using an autograft, the surgeon carefully measures and removes the skin from the donor site. Sometimes the graft is cut in a mesh-like pattern, to allow a small piece of donated skin to stretch and cover a larger area. This also helps oxygen pass through the graft. The site where the skin graft will go is cleaned well to remove any dirt, bacteria, or dead skin cells. Then the skin graft is placed and attached with stitches or staples. It is often covered with antibacterial ointment and supportive bandages for the first few days.

As the skin graft heals into its new position, new blood vessels grow to reach and support the skin. Depending on the thickness of the graft, oil glands, sweat glands, and hair follicles may also regrow. Recovery time depends on the thickness of the graft and the overall health of the skin in the grafted area, but it generally takes from two to four weeks.
