We created our clinic with the mission to improve population health, enhance patient experience of care and reduce the rate of cost of care.

We have a dedicated CareTeam and Medical Support staff to identify resources that are essential to improve your health and well-being. It’s one clinic built as a community to encourage long-term relationships with our patients, and care for you from birth through end of life.

New Patients

We are accepting new patients including Oregon Health Plan, Medicare and HealthShare patients. We are currently covered with most private insurers, accepting Motor Vehicle and Workers Compensation Claims as well.
