San Francisco Community Clinic Consortium is a partnership of 12 community health centers that together form the largest provider of primary care safety net services in San Francisco. For over 40 years, SFCCC and our members have worked to improve the health and well-being of people across the city.

Our member Clinics

Our 12 member clinics care for more than 110,000 San Franciscans each year, many of whom face challenges accessing healthcare. Our clinics are located across the city to provide services where they are most needed.

In addition to comprehensive primary and preventive care, many clinics offer a range of services in one location, including dental and vision care, behavioral health support, and health education.
