Prescription sous contrôle médical

Action thérapeutique

• Vitamine

Indications

• Traitement initial des carences sévères en thiamine (vitamine B1) : formes aiguës sévères de béribéri, complications neurologiques de l’alcoolisme chronique (delirium tremens, encéphalopathie de Wernicke)

Présentation

• Ampoule à 100 mg de thiamine chlorhydrate (50 mg/ml, 2 ml) pour injection IM ou IV très lente (en 30 minutes)

Posologie

Béribéri infantile 25 mg en IV, puis 25 mg en IM une à 2 fois par jour ; prendre le relais par voie orale (10 mg par jour) le plus rapidement possible, dès l’amélioration des symptômes.

Béribéri aigu 50 mg en IM puis prendre le relais par voie orale (50 mg 3 fois par jour jusqu’à l’amélioration des symptômes puis 10 mg une fois par jour) ou, selon la sévérité, 50 mg en IM toutes les 8 heures pendant quelques jours puis prendre le relais par voie orale (10 mg une fois par jour)

Delirium tremens, encéphalopathie de Wernicke 100 mg en IM ou IV 3 fois par jour pendant 3 à 5 jours

Contre-indications, effets indésirables, précautions

• Peut provoquer : hypotension ; réaction anaphylactique, en particulier lors de l’injection IV (injecter très lentement, en 30 minutes).
• Grossesse : pas de contre-indication
• Allaitement : pas de contre-indication

Remarques

• La thiamine est aussi appelée aneurine.
• La thiamine injectable ne fait pas partie de la liste des médicaments essentiels de l’OMS.
