You need to have 2500 Simoleons, ten garlic, ten plasma, and ten wolfsbanes to make the drink. You can order garlic straight from your computer and then open up the packets in your inventory. For plasma food and wolfsbane, you can find them by searching around the area of Forgotten Hollow, as shown in the image below.

You can purchase a bar from the store, and then you have to mix all the ingredients and make the ultimate vampire cure drink. After that, drink it, and you can share the drink with other vampires as well. When you drink it, you will turn back into a regular Sim.
