The Integrated Decision Support (IDS) system at Hamilton Health Sciences has allowed for improved collaboration across health care sectors. It allows users to have an increased understanding of the hospital, community care access centre, community health centre, and mental health services, and how change in one sector influences the others. Improved patient flow, improved Alternate Level of Care ALC rates, streamlined access, and a better understanding of chronic conditions and readmission rates can all lead to an improved patient experience. The openness and collaboration that results among health care professionals who work together, is an intangible yet critical impact.
