Cả bốn loại ký sinh trùng sốt rét trên tuy có khác nhau về hình thái học nhưng diễn biến chu kỳ ở người và muỗi truyền bệnh tương tự nhau, gồm 2 giai đoạn:[3]

• Giai đoạn sinh sản và phát triển vô tính trong cơ thể người.
• Giai đoạn sinh sản hữu tính ở muỗi Anopheles truyền bệnh.

Trong đó người là vật chủ phụ, muỗi là vật chủ chính, thiếu một trong 2 vật chủ này thì Plasmodium không thể sinh sản và bảo tồn nòi giống được.

Giai đoạn sinh sản vô tính trong cơ thể người

Giai đoạn sinh sản vô tính trong cơ thể người chia làm hai thời kỳ, thời kỳ phát triển trong gan và thời kỳ sinh sản vô tính trong hồng cầu. Quá trình cụ thể như sau: muỗi Anopheles mang mầm bệnh (thoa trùng) đốt người, thoa trùng từ nước bọt của muỗi truyền vào máu ngoại biên của người. Thoa trùng chủ động tìm đường xâm nhập vào gan, vì tại giai đoạn đó máu không phải là môi trường thích hợp cho thoa trùng tồn tại và phát triển, thời gian chúng ở trong máu chỉ dưới 1 giờ đồng hồ.

Thoa trùng xâm nhập tế bào gan và bắt đầu phân chia, đến một lượng nhất định làm tế bào gan bị vỡ ra giải phóng những ký sinh trùng thế hệ mới, đây là giai đoạn phát triển của nhiều thoa trùng. Từ gan vào máu, ký sinh trùng xâm nhập hồng cầu, chúng sinh sản vô tính tại đây đến mức độ đầy đủ làm vỡ hồng cầu giải phóng ký sinh trùng, đại bộ phận những ký sinh trùng này sẽ lại thâm nhập vào hồng cầu khác để tiếp tục sinh sản vô tính.

Nhưng một số mảnh ký sinh trùng khác trở thành những thể giao bào đực cái, nếu muỗi hút những giao bào này, chúng sẽ phát triển chu kỳ hữu tính ở trong dạ dày của muỗi, nếu không được muỗi hút thì sau một thời gian sẽ bị tiêu hủy. Khoảng thời gian cần thiết để hoàn thành chu kỳ vô tính trong hồng cầu tùy từng chủng loại Plasmodium, có thể từ 40 đến 72 giờ, đo đó trong khoảng thời gian này cơ thể người thường bị sốt rét cách nhật.

Giao bào đực và cái được muỗi hút vào dạ dày sẽ phát triển thành những giao tử đực và cái, qua sinh sản hữu tính sinh ra thoa trùng. Các thoa trùng đến tập trung trong tuyến nước bọt của muỗi là tiếp tục truyền bệnh cho người khác.
